The close cooperation with the Childrens Hospital offers you the unique opportunity to see urethral reconstruction in adults as well as in childrens during your stay. For hypospadias repair the following techniques are used: modifications of the Nesbit procedure to correct curvature; for urethral reconstruction: meatoplasty, glanuloplasty, pedicled fore-skin flap, buccal mucosa urethroplasty one and two-staged. For urethral reconstruction in adults, end-to-end anastomosis, pedicled flaps, buccal mucosa inlay and onlays as well as the two-staged meshgraft-technique are used. We will provide you with theoretical background. You might also see a complex reconstruction as open reanastomosis for postprostatectomy anastomotic stricture or closure of recto-urethral fistula and learn about the treatment of complications after treatment for prostate cancer. Any type of urinary diversion (neobladder, sigma-rectum pouch, ileocecal pouch, transverse pouch, ileal or colonic conduit) is performed at Hamburg University.

Beside reconstructive urology there are other interesting field covered by further staff members of the Department. The Martini Clinic and the University of Hamburg perform together more than 1500 radical prostatectomies per year. The Da Vinci system is regularly used. Laparoscopy for organ sparing tumor surgery, pyeloplasty and nephrectomy belongs to the armamentarium. For BPH laser enucleation (HOLEP) is available. Minimal invasive stone treatment (mini-PCNL, ureterorenoscopy) is a part of the routine spectrum.

Students will be integrated into the team and will shadow our Chief Resident. During the 3 month attachment the student will have in depth insight into the logistics of a busy full-time reconstructive urology service of a tertiary referral centre. The student will become familiar with all techniques currently used in urethroplasty and will also learn about the surgery of the complications of the treatment of prostate cancer (surgery/radiotherapy/HIFU/cryotherapy) such as artificial sphincter implantation, urorectal fistula repair, redo-vesico-urethral anastomosis, salvage prostatectomy and various other reconstructive and urinary diversion procedures.

Additionally, students will gain experience with our Andrology firm. David Ralph and his colleagues offer one of the most distinguished andrology services worldwide with all aspects of andrological care and gender reassignment. About 100 penile prostheses are implanted per year.

During the course students will be able to fully evaluate the patient with urethral stricture disease, be able to perform retrograde and voiding, urethrography, urethral sonography, and endoscopic evaluation of the patient with urethral stricture disease, be familiar with all of the surgical techniques suggested for the repair of urethral stricture disease, be able to perform the harvesting of all tissues used for urethroplasty: oral mucosa (cheek, lip or tongue), genital and extragenital skin, be familiar with the literature on reconstructive urethral surgery and be able to write an article on reconstructive urethral surgery.
